Child and Family Therapist - Home-Based

Job Summary

Conduct assessment, diagnosis, treatment planning, and individual/family/group therapy for a caseload of 10-15 youth (ages 7-21) with mental health or co-occurring substance use needs. Deliver services in the home, community, or via tele-health, including direct face-to-face contact of at least 6 hours monthly. Coordinate cases with psychiatric staff, school personnel, and other treatment team members while obtaining, analyzing, and evaluating case data to develop and adjust treatment goals. Maintain clinical records in the Electronic Medical Record in accordance with OCHN and MDHHS standards. Participate in weekly supervision, specialized training groups, and required licensure trainings. Provide referrals for social determinants of health and sign up for crisis and check-in coverage rotations.

Required Qualifications

  • Successful completion of the requirements of a master's degree from an accredited school of social work or equivalent in related field such as psychology, social work, or guidance and counseling as appropriate to the needs of the particular position.
  • State certification or licensure, commensurate with accrued experience and degree obtained (LLMSW, LMSW, LLPC, LPC, TLLP, LLP, LLMFT, LMFT).
  • At least one year of experience in the examination, evaluation, and treatment of minors and their families pre- or post-master's degree.
  • Experience working in the Community Mental Health System.
  • Substance use disorder qualifications.
  • Experience with youth with serious emotional disturbances.
  • Family systems work.
  • Trauma-informed care.
  • Delivery of home-based models.
  • Use of evidenced based models.
  • Sensitivity to the diversity of the agency's service population including (but not limited to) differences of culture, race, religion, gender, sexual orientation and socioeconomic.
  • Documentation of personal vehicle liability insurance with minimum coverage of $100,000 per person/ $300,000 per incident.
  • Valid driver's license on an annual basis as well as proof of registration.
  • Expect to work at minimum two evenings a week to accommodate school and work schedules.
  • Participate in the SSY crisis on-call coverage for individuals by signing up for at least 3-4 weeks per year.
  • Participate in the SSY check-in coverage for individuals by signing up for at least 3-4 weekends per year.
  • Attend weekly department meetings, trainings and supervision scheduled.
  • Attend specialized supervision groups for TF-CBT, SUD, licensure and high-risk cases as assigned by supervisor.
  • Maintain clinical records in accordance with Agency, licensing, accreditation, Oakland Community Health Network (OCHN), Michigan Department of Health and Human Services (MDHHS) standards and requirements.
  • Maintain all records in the Electronic Medical Record (EMR) within timeframes required by OCHN.
  • Provide referrals or assistance with the social determinants of health, including physical health, cultural, employment, educational, social or financial needs.
  • Responsible for incorporating principles of diversity, equity and inclusion in work processes and in job functions.
  • Responsible for promoting a feeling of welcoming, belonging, and acceptance to all clients and staff.

Desired Qualifications

  • Other special abilities, skills and credentials such as experience working in the Community Mental Health System, substance use disorder qualifications, experience with youth with serious emotional disturbances, family systems work, trauma-informed care, delivery of home-based models and use of evidenced based models.
  • Special abilities and skills necessary to perform the required tasks and that best meet the needs of the agency also will be considered.
  • There may be some standards above that may be waived when compensating specifications or circumstances exist if they do not preclude state Medicaid requirements to provide Home Based services.
