Job Summary
Coordinate and conduct team processes to develop comprehensive Individualized Treatment Plans for clients and families. Work closely with Clinical Professionals to deliver services based on prescribed curriculum lesson plans. Assess client, family, and environments using assessment tools while spending time with assigned individuals as identified on treatment plans. Provide direct support to assist in gaining access to medical, social, educational, vocational, and recreational services. Requires a BS/BA in Human Services and strong communication skills. Earn $19.75 - $21.92 per hour.
Required Qualifications
- A thorough understanding of human services, resources, and tools
- The ability to assess clients (and families) from a strengths model perspective
- Skills, knowledge, and abilities to develop a theoretical perspective
- The ability to work with a variety of people
- Strong oral and written communication skills
Desired Qualifications
- A BS/BA Degree in a Human Services Related Field
- An equivalency based on documented work history, education, and volunteer experience
