Job Summary
Provide community-based case management, skills training, and developmental support to children and their families across three counties. Conduct family, individual, and group counseling while delivering emergency crisis intervention as needed. Manage treatment plans, coordinate referrals, and attend collateral meetings with all parties involved in the process. Maintain accurate records reflecting patient progress and meet daily production standards for chart management and patient contact. Participate in regular staff meetings and ongoing training activities. Additionally, conduct consultation and education for community and professional groups to enhance public awareness. This role requires a valid driver's license for daily travel and involves background checks including CPS and sex offender registry verification.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice or other related degrees
- Must possess the ability to assess emergency situations and react in an appropriate and therapeutic manner
- Must have a valid driver's license
- Must have auto insurance that meets the Indiana State minimum coverage requirement
- Must qualify for Company's vehicle liability insurance
- Must be able to travel daily throughout the 4 counties we serve
- Must pass required background checks including county/state checks
- Must pass fingerprinting
- Must pass CPS check
- Must pass sex offender registry check
- Must pass drug screen
