Job Summary
Provide case management services to clients; collect data from collateral sources; review relevant records and other pertinent data and resources; participate as a member of a treatment team in the planning, formulation, implementation, and review of treatment, Personal Support or Individual Service Plan; provide services to identified children and families in order to assist in gaining access to medical, social, educational, vocational, and recreational services; operates within a supportive and inclusive team environment with opportunities for professional growth and competitive benefits.
Required Qualifications
- BS/BA Degree in a Human Services Related Field preferred or equivalent experience
