Job Summary
Provide case management services to clients by collecting data from collateral sources and reviewing relevant records. Participate as a member of a treatment team in the planning, formulation, implementation, and review of treatment, Personal Support, or Individual Service Plans. Assist identified children and families in gaining access to medical, social, educational, vocational, and recreational services. A thorough understanding of human services and the ability to assess clients from a strengths model perspective are required. A BS/BA Degree in a Human Services Related Field is preferred. AWARE offers competitive compensation ranging from $20.00 to $22.19 per hour, along with health insurance, dental/vision coverage, a 401(k) match, and a tuition advancement program.
