Job Summary
Provide case management services to clients by collecting data from collateral sources and reviewing relevant records. Participate as a member of a treatment team in the planning, formulation, implementation, and review of treatment, Personal Support, or Individual Service Plans. Assist identified children and families in gaining access to medical, social, educational, vocational, and recreational services. Requires a BS/BA in a human services field and strong communication skills. Earn $20.00 - $22.19 per hour with health, dental, vision, and 401(k) benefits.
Required Qualifications
- A thorough understanding of human services, resources, and tools
- The ability to assess clients (and families) from a strengths model perspective
- Skills, knowledge, and abilities to develop a theoretical perspective
- The ability to work with a variety of people
- Strong oral and written communication skills
Desired Qualifications
- A BS/BA Degree in a Human Services Related Field
- An equivalency based on documented work history, education, and volunteer experience
