Job Summary
Deliver in-home and community-based psychiatric rehabilitation to children and adolescents (ages 5 to 17) and their families. Conduct assessments (including the CANS) and jointly build Individualized Service Plans (ISPs) with youth and families. Provide skills training, crisis intervention, and family-centered support in the home. Collaborate with caregivers, schools, therapists, and other providers. Participate in a 24/7 on-call rotation for crisis response and home visits. Maintain timely, accurate documentation in line with COMAR and program standards.
Required Qualifications
- Bachelor's degree in human services field or related
- Active CPRP (Certified Psychiatric Rehabilitation Practitioner)
- More than 2 years of behavioral health experience
- Experience working with children, adolescents, and families
- Valid driver's license and reliable transportation
- Willingness to travel for in-home visits
- Ability to pass COMAR 10.63.01.05 background check
