Job Summary
Certified Home Health Aide (CHHA) with CNA/HHA credentials to support hospice patients in home settings under RN supervision. Responsibilities include performing personal care, assistance with activities of daily living, meal preparation, medication reminders as allowed by state regulations, documenting patient condition, ensuring safety, and collaborating with the interdisciplinary team to implement the plan of care. Requires strong communication, empathy, ability to work autonomously, and readiness to travel to patients’ homes in Stockton, CA.
Required Qualifications
- Certified Nursing Assistant (CNA) in good standing
- California Home Health Aide (HHA) certification in good standing
- Willingness to undergo criminal background check
- Current CPR certification
- Licensed driver with insured vehicle
- Experience preferred in community health/hospice or medical/surgical environments
- Completion of minimum 75 hours of classroom and supervised practical training
