Job Summary
Certified Home Health Aide position supporting hospice patients in home settings. Work under the supervision of a Registered Nurse to provide personal care and comfort, assist with activities of daily living (bathing, dressing, feeding, mobility), monitor and report patient condition, maintain safety, and collaborate with the interdisciplinary team to carry out the patient care plan. Strong communication with patients, families, and healthcare providers; compassionate, autonomous in-home care; adherence to care procedures and documentation; CPR and background check prerequisites; valid CNA and California HHA certifications required. Willingness to participate in quality improvement activities and meet organizational standards for care.
Required Qualifications
- Must be a Certified Nursing Assistant (applicable for all States, additional requirements for California residents, see below).
- California REGULATION: Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California.
- Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
- Must have completed a minimum of 75 hours of classroom and supervised practical training
- Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
- Willingness to submit to a criminal background check prior to hire and annually thereafter
- Current CPR certification
