Job Summary
Receive and provide shift reports to management on resident status, notifying them of any changes in physical or mental condition. Deliver personal care per individualized plans, including ADLs like bathing, grooming, turning, and positioning. Prepare residents for meals, assist with dining, and manage toilet and incontinence care. Perform clinical procedures, vital signs, and prevention/restorative activities such as PROM, AROM, and ambulation. Maintain resident apartments, conduct room checks, and adhere to infection control and emergency protocols. Participate in interdisciplinary team processes, care plan meetings, and quality assurance programs while communicating with families and staff. Maintain education compliance and confidentiality.
Required Qualifications
- High School Diploma or equivalent
- six months of experience in a caregiving role or similar function
- CNA or HHA
- Excellent verbal and written communication skills
- Able to communicate verbally in person and over the phone
- Must be available for weekend shifts
- Must be available for 11pm - 7am shifts
- Must be available for every other weekend
- Working knowledge of facility emergency procedures including fire and disaster drills
- Maintains resident confidentiality and adheres to HIPAA policies
- Maintains a working knowledge of the organization in order to answer basic questions
- Possesses working knowledge of facility infection control procedures
- Maintains education development and compliance with State and Federal Regulations and current knowledge of resident care practices
Desired Qualifications
