Job Summary
The Chart Analyst functions as a therapist and Qualified Mental Health Professional (QMHP) for clients, ensuring that assessments and treatment plans are clinically relevant and timely. The role involves ongoing documentation of clinical services, coordination with the treatment team, preparation for weekly clinical meetings, and reviewing an average of 85 charts per month to maintain compliance with SCHARP standards. Responsibilities also include generating monthly reports for the Program Director regarding chart reviews.
Required Qualifications
- Functions as QMHP for assigned clients
- Completes Adult Initial Assessment, Treatment Plans, and Tri-Annual Assessments
- Maintains up-to-date DMH clinical services documentation
- Coordinates with treatment team
- Prepares for clinical staff meetings and supervision
- Reviews charts to ensure documentation meets SCHARP standard
- Provides monthly report to Program Director
