Job Summary
Charge Nurse LVN position in Denison, TX, providing licensed nursing care in accordance with each resident’s care plan and interdisciplinary team directions. Responsibilities include RAIs and census accuracy, coordinating transfers/discharges, pre-certifications/recertifications for Medicare Advantage and commercial payers, maintaining policies and procedures, participating in facility surveys and committees, charting and documentation, medication administration, and staff development. The role emphasizes dementia care planning, culturally competent and trauma-informed care, and collaboration with IDT to ensure safe, person-centered care. Requires an LPN/LVN license or a nursing degree, up to one year of related experience, and ability to work full-time on a day shift; shift is 6:00 AM to 6:00 PM in a Denison facility with an emphasis on safety, quality assurance, and patient-focused care.
Required Qualifications
- Nursing degree from an accredited college or university
- Graduate of an approved Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) program
- Up to one year of experience in hospital, nursing care facility, or related health care facility
- Training in rehabilitative and restorative nursing practices
- Ability to provide care appropriate to the age-related needs of residents
- Demonstrate teamwork and collaborative skills
- Ability to make independent decisions when warranted
- Effective communication with residents, families, and staff
- Patience and tact in handling residents and families
- Willingness to learn and incorporate new nursing practices
- Respect for residents' rights and choices
- Ability to push/pull/move residents and lift at least 25 pounds
- Not pose a direct threat to health and safety
