Chaplain / Spiritual Counselor - Hospice - FT

Job Summary
Assess spiritual and emotional needs of patients, caregivers, and families to develop individualized care plans. Provide counseling, grief support, and compassionate presence during end-of-life transitions while collaborating with nurses, physicians, and social workers in interdisciplinary team meetings. Coordinate visits from local clergy and maintain accurate clinical documentation aligned with hospice standards. Conduct field-based visits in homes, facilities, and community settings with high autonomy. Requires a theological degree (BD, MDiv) and experience in hospice or clinical pastoral education.
Required Qualifications
- Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree required)
- Knowledge of and commitment to hospice philosophy of care
- Ability to work effectively within an interdisciplinary team environment
- Comfort serving within an interfaith setting
- Ability to support individuals from diverse cultures, lifestyles, beliefs, and backgrounds
- Excellent communication, empathy, and active listening skills
- Experience working with death, dying, grief, and family/caregiver support
- Valid driver's license, reliable transportation, and current automobile insurance for field-based responsibilities
Desired Qualifications
- Hospice, palliative care, or healthcare spiritual care experience
- Clinical Pastoral Education (CPE) experience
- Strong grief counseling and emotional support skills
- Ability to quickly build trust and meaningful relationships
- Established relationships with community clergy and faith-based organizations
- Experience documenting within an EMR platform
