Job Summary

Independently manage an assigned caseload of hospice patients by determining visit frequency, scheduling, and pastoral approach based on autonomous spiritual and clinical assessments. Conduct individualized spiritual evaluations to identify needs across all faith traditions and provide professional counseling, crisis intervention, and real-time emotional support. Participate as a peer-level contributor in the Interdisciplinary Group to present care updates alongside nursing and clinical leadership, while independently managing bereavement support relationships following patient death. Collaborate with team members to resolve spiritual, pastoral, and ethical issues, ensuring full compliance with federal regulations and internal policies. Requires advanced pastoral knowledge, current driver's license, and proficiency with EMR software including FireNote.

Required Qualifications

  • Advanced pastoral knowledge and competency
  • Bachelor of Divinity, Master of Divinity, or equivalent graduate theological degree from an accredited seminary or school of theology
  • Minimum one (1) unit of CPE (400 supervised hours) from an ACPE-accredited program
  • Board Certified Chaplain (BCC) or equivalent credential through APC, NACC, NAJC, or ACPE
  • Ordination by a recognized religious body
  • Minimum of three (3) years of active pastoral ministry with demonstrated grief, bereavement, or death and dying ministry experience
  • Documented pastoral leadership, crisis counseling, or hospice/healthcare chaplaincy experience
  • Minimum one (1) year of experience in a healthcare setting or with death and dying
  • Demonstrates comfort working in an interfaith and multi-belief setting
  • Ability to provide spiritual care without imposing personal religious beliefs
  • Current State Driver's License
  • Reliable transportation
  • Valid auto liability insurance
  • Proficiency with computer systems including EMR software
  • Strong written and verbal English communication skills
  • Demonstrated ability to manage an independent caseload with minimal supervision

Desired Qualifications

  • Four units of CPE
  • Hospice or home health experience
  • Experience with FireNote
