Job Summary
Conduct spiritual assessments for patients, families, and caregivers to ensure timely service delivery and facilitate individualized care plans. Provide direct spiritual care, document service utilization for quality assurance, and serve as a liaison to community chaplains and spiritual counselors. Recruit and manage spiritual care volunteers, while offering consultation and education to interdisciplinary groups. Plan memorial services and actively participate in quality assessment and performance improvement teams.
Required Qualifications
- Graduate of an accredited seminary or school of theology
- BD
- M.Div. or equivalent theological degree
- Experience working with death and dying, individuals/family/caregiver
- Willingness to travel to hospice locations as assigned
- Flexibility with working hours
Desired Qualifications
- Hospice experience
- Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems
