Job Summary
Evaluate spiritual needs of patients and families referred by the Hospice team and perform home visits to provide spiritual support and counseling services. Function as a liaison to personal or religious advisors and work cooperatively with all disciplines to facilitate the interdisciplinary team approach. Participate in Bereavement Program activities, Hospice Interdisciplinary Group meetings, individual care conferences, and QI initiatives as requested. Document care and services per agency policies while maintaining professional expertise through educational activities and community engagement.
Required Qualifications
- A religious professional
- A combination of documented formal training in pastoral counseling and supervised counseling
- Experience in a health care or clinical setting
- Academic and supervised work experience must equal at least five years
- Excellent verbal and written communication skills
- Bachelors level degree
- One unit of ACPE Certified CPE
- Must be able to lift 50 lbs
- Must be available for weekend shifts
- Valid driver's license
Desired Qualifications
- Previous experience in Hospice or Home Care preferred
- Masters level degree in Theology/Ministry or related field
