Job Summary
Assure spiritual assessment of patients and families in the hospice program and facilitate individualized care plans by participating in comprehensive assessments. Provide direct spiritual care, serve as a liaison to community chaplains, and maintain records for quality assurance and policy review. Attend interdisciplinary meetings to provide consultation and education on spiritual care needs. Recruit community chaplains and volunteers by developing contacts and offering congregational education on hospice care. Plan periodic memorial services and assist in supervising spiritual care volunteers when assigned. Provide funeral or memorial services as requested.
Required Qualifications
- Graduate of accredited seminary or school of theology
- Experience working with death and dying, individuals/family/caregiver
- Ability to work as member of interdisciplinary team/group
- Comfort in an interfaith setting
- Ability to accept different lifestyles, cultures, beliefs, and values
- Ability to network with community clergy and congregations
- Knowledge of and commitment to hospice philosophy of care
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
Desired Qualifications
- Preferred (BD, MDiv or equivalent theological degree)
- Hospice/ experience preferred
