Chaplain - Main Palliative Care - PRN

Chaplain role providing spiritual and emotional care to chronically and terminally ill patients using the Palliative Care Transdisciplinary Model. Collaborates with a transdisciplinary team of doctors, nurses, social workers, pharmacists, patients, families, and caregivers to develop and implement a plan of care aimed at promoting patient well-being, continuity of care, quality of life, and end-of-life goals. Facilitates advance directives (living wills and health care powers of attorney) and provides information to patients and families about community resources.
