Chaplain – Hospice - Part-Time

Job Summary
Conduct spiritual assessments and develop individualized care plans for patients and families navigating serious illness. Provide ongoing emotional support aligned with beliefs, traditions, and preferences, offering prayer, sacraments, or rituals as requested. Participate in interdisciplinary group meetings to contribute to holistic care planning and assist with advance care planning or legacy work. Document all services and plan updates in the EMR while coordinating with community clergy. Support bereavement care before and after death, including memorial services and grief follow-up. Educate team members on cultural and ethical considerations while maintaining confidentiality. Perform other duties as assigned.
Required Qualifications
- Master's degree in divinity, theology, pastoral counseling, or a related field from an accredited institution
- Minimum of one unit of Clinical Pastoral Education required
- Ordination, commissioning, or endorsement by a recognized faith group
- Valid driver's license and reliable transportation
Desired Qualifications
- two units or board certification preferred
- Experience providing spiritual care in hospice or healthcare settings preferred
- Understanding of grief and bereavement processes
- Knowledge of Medicare hospice regulations and Conditions of Participation
- Strong interpersonal and communication skills
- Ability to work independently and collaboratively
- Proficiency with EMR systems and virtual communication tools
