Job Summary

Conduct timely spiritual assessments for patients and families, then participate in developing individualized care plans to address spiritual needs. Provide direct spiritual support to patients, families, and caregivers while serving as a liaison to community chaplains and spiritual counselors. Maintain accurate documentation of spiritual services and communications, and attend interdisciplinary meetings to contribute to care planning. Offer education and spiritual care guidance to the care team, recruit and support community chaplains and volunteers, and coordinate memorial or funeral services as requested. Organize periodic memorials for staff, volunteers, and community partners, and assist in supervising spiritual care volunteers. This role offers full-time Monday-Friday 8AM-5PM coverage in the Jackson/Sheboygan area, with benefits including medical, dental, vision, and tuition reimbursement.

Required Qualifications

  • Graduate of an accredited theology or chaplaincy program (BD, MDiv or equivalent theological degree)
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
  • Comfort in an interfaith setting
  • Ability to accept different lifestyles, cultures, beliefs and values
  • Knowledge of and commitment to hospice philosophy of care
  • Ability to work as member of interdisciplinary group
  • Ability to network with community clergy and congregations

  • Experience working with death and dying
