Job Summary
The Chaplain provides spiritual counsel and support to hospice patients and their families; develops and implements a plan of care with the interdisciplinary team, the patient, and family, including the patient’s faith community if requested; visits patients and families, offering spiritual care; engages in prayer and sacramental ministry; plans and conducts funerals or memorial services; assists patients/families in crisis; communicates with clergy or faith communities; provides bereavement support and staff education on beliefs and rituals; participates in care planning conferences, memorial services, and on-call rotations; complete clinical documentation per Alive Hospice procedures; participates in department meetings and education sessions.
Required Qualifications
- Master of Divinity Degree or equivalent graduate degree that provides advanced education to provide spiritual care and counseling
- Minimum of two (2) units of Clinical Pastoral Education (CPE) required
- Minimum two years’ work experience
- Three years of experience in Pastoral Care strongly preferred
