Job Summary
Conduct spiritual assessments and care for patients and families, facilitating individualized plans of care through comprehensive interdisciplinary meetings. Serve as a liaison to community clergy, recruiting volunteers and providing consultation on spiritual needs while documenting services for quality assurance. Plan funeral and memorial services as requested and supervise spiritual care volunteers when assigned. Participate in performance improvement teams and adhere to company policies, maintaining records for program development.
Required Qualifications
- Graduate of accredited seminary or school of theology (Bachelor of Divinity, Master of Divinity or equivalent theological degree)
- Chaplain Pastoral Education (CPE) Units (Varies by State)
- Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order
- Must understand and follow company policies and procedures
- Must be able to network with community clergy and congregations and present a professional image
- Experience working with death and dying, individuals/family/caregiver
- Ability to work as member of interdisciplinary group
- Comfort in an interfaith setting
- Ability to accept different lifestyles, cultures, beliefs, and values
- Knowledge of and commitment to hospice philosophy of care
- CPR Certification
