Job Summary
Field-based CFSP Managed Care Coordinator in North Carolina counties (Alamance, Orange, Guilford, Chatham, Caswell) focused on integrated whole-person care management across physical health, behavioral health, I/DD, LTSS, and pharmacy; collaborates with county Child Welfare Workers, multidisciplinary teams, and caregivers to coordinate care, set goals, and address unmet health-related needs; conducts telephonic or face-to-face assessments, identifies members with clinical needs, coordinates care with clinical teams, and oversees non-clinical needs for members with chronic illnesses or disabilities; role emphasizes Trauma-Informed Care and alignment with permanency planning goals, with requirements to travel to client sites four to five days per week and potential office attendance; NC residency and reasonable commuting distance required, with a hybrid work policy allowing some flexibility.
Required Qualifications
- Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the specific program population or other related community-based organizations; or any combination of education and experience which would provide an equivalent background.
Additional Requirements
- Must reside in North Carolina; travel to worksites and other locations as necessary; field-based role with some office attendance; hybrid/virtual work policy requires commuting distance within NC or accommodation if applicable.
