Certified Social Worker needed for Adult Medical Day Care team; provide social assessments, Medicaid/benefits coordination, care plan development, and assistance with housing and billing processes. 8:00 AM–4:00 PM, full-time with a $25/hour wage and $500 signing bonus. No weekends or evenings. Experience in Adult Medical Day Care or long-term care is a plus. Responsibilities include interviewing participants and caregivers, completing social assessments, verifying Medicaid eligibility, assisting with applications and renewals, and support with related paperwork and problem resolution.
