Job Summary
Certified Social Worker needed for an Adult Medical Day Care program. Responsibilities include interviewing participants and caregivers for social assessment and identifying needs, completing government documentation for New Jersey adult daycare, quarterly verification of Medicaid eligibility, assisting with Medicaid, food stamps, housing applications and renewals, developing quarterly care plans, and helping participants with medical and other billing issues.
Required Qualifications
- Certified Social Worker
- Adult Medical Day Care experience is a plus
- Ability to interview participants and caregivers for social assessment
- Experience with Medicaid/Medicare documentation and eligibility verification
- Ability to assist with government applications and renewals
- Care plan development
- Billing problem resolution for participants
