Job Summary
Entry-level MSW role providing psychosocial assessment, care planning, and counseling to dialysis patients, collaborating with the interdisciplinary health care team to promote rehabilitation and quality of life. Responsibilities include conducting psychosocial assessments, participating in care planning, utilizing education programs and interventions, supporting transplant referrals, connecting patients with community resources, addressing advance directives, and contributing to Quality Assessments & Improvement (QAI) activities. Requires MSW and state licensure; 0-2 years of related experience. Familiarity with ESRD patient education, quality indicators, and coordination with insurance and administrative processes is expected.
