Job Summary
Conduct case management and community support services by managing a caseload and completing needs assessments for mental health, substance use, and other barriers to recovery. Engage individuals through outreach in various settings including homes, community agencies, and healthcare providers to empower self-directed recovery using tools like WRAP. Maintain knowledge of housing, transportation, and treatment resources while advocating for reasonable accommodations and access to benefits. Work independently and within a team-based environment to promote the recovery model and trauma-informed care.
Required Qualifications
- Minimum high school diploma or equivalent
- 21 years of age
- Certified Recovery Support Specialist (CRSS)
- Must be willing to obtain CRSS within 1 year of the date of hire
- Individuals who are willing to self-identify as having lived experience of a mental health or substance use diagnosis
- Valid driver's license
- Reliable transportation
- Proof of current insurance
Desired Qualifications
- Certified Recovery Support Specialist (CRSS) preferred
