Job Summary
Conduct patient outreach, medication reconciliation, and adherence monitoring for Medication Therapy Management services. Gather comprehensive medication lists through patient interviews and provider records, then document therapy problems and interventions in MTM software for pharmacist review. Coordinate follow-up calls regarding therapy changes, lab tests, or refills while maintaining compliance with HIPAA regulations, Medicare Part D requirements, and company policies. Identify qualifying patients and schedule appointments with providers and residents for medication reviews. Requires active Tennessee Board of Pharmacy Registration and one to three years of pharmacy technician experience.
Required Qualifications
- Active Tennessee Board of Pharmacy Registration
- Previous Long Term Care Pharmacy experience
- One to three years of pharmacy technician experience
- Proficiency with information systems
- Microsoft Office
- Must possess required skills and be able to explain and demonstrate, with or without reasonable accommodations, that the essential functions of the job can be performed
- Excellent communication and customer service skills
- Maintaining an established work schedule
- Effectively using interpersonal and communications skills including tact and diplomacy
- Effectively using organizational and planning skills with attention to detail and follow through
- Establishing and maintaining effective working relationships
- Effectively demonstrates accuracy and thoroughness and continually strives to improve quality standards
- Maintaining confidentiality of work related information and materials in compliance with HIPAA regulations
- Adheres to the principles of the Employee Standards and Code of Conduct and reports any violations
- Occasional lifting of objects up to 50 pounds
- Must have reliable transportation
Desired Qualifications
- Pharmacy Technician Certification (CPhT) preferred
- Completion of pharmacy technician program preferred
- Or equivalent combination of education and/or experience
