Job Summary
Visit members and their families in the Bronx, Brooklyn, and Queens to identify engagement opportunities and build supportive recovery networks. Provide onsite psychiatric discharge planning education at Recovery and Resiliency sites while collaborating with Stabilization Teams to resolve barriers and facilitate service transitions. Maintain contact with assigned individuals and providers to monitor member responses to services. Requires New York Certified Peer Support Specialist certification and at least two years of experience in health or behavioral health. Salary ranges from $23.31 to $34.97 for this 1st shift field role.
Required Qualifications
- New York Certified Peer Support Specialist
- AA/AS and minimum of 2 years of experience in health services or behavioral health field or in a peer support services role
- Valid, active, certified recovery specialist/coach certification
- peer/family support specialist certification
- COVID-19 vaccination
- Influenza vaccination
Desired Qualifications
- Knowledge of care-coordination and case management concepts
- BA/BS or MBA
