Job Summary

Deliver peer support, psychosocial rehabilitation, and family services to CORE program members through one-on-one interactions and goal-oriented advocacy. Manage caseloads by overseeing individualized service plans, coordinating transportation, and accompanying clients to behavioral health and medical appointments. Facilitate Saturday programming for the PROS initiative while ensuring documentation compliance and participating in interdisciplinary team meetings. Provide health coaching on medication adherence and wellness self-management, acting as a role model for participants within a trauma-informed framework.

Required Qualifications

  • High School diploma or GED
  • Active NYS Peer Certification (NYCPS) or NYS Peer Certification-Provisional (NYCPS-P) credential
  • Lived experience living with a disability
  • History of mental health challenge
  • History of substance abuse
  • Willingness to draw upon lived experience to serve as a role model
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
  • Ability to write comprehensive reports and documentation
  • Ability to speak effectively before groups of customers or employees of organization
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs
  • Knowledge of MS Word, Excel and internet software
  • Daily access to an automobile
  • Valid NY State Driver's License
  • Acceptable driving record for agency insurance coverage
  • Clearance under the NYS Justice Center and NYS OMH mandated criminal background check process
  • Clearance under the Medicaid Fraud & Abuse clearance process
  • Regular work schedule: Up to 15 hours per week to be scheduled from Mondays to Fridays between the hours of 8:30am and 4:30pm plus an additional 5 hours per week on Saturdays between 9:00am and 2:00pm
