Job Summary
Use lived experience to inspire hope, strengthen engagement, and support participants in their recovery journeys. Partner collaboratively with participants, families, and team members to manage an assigned caseload and serve as chart manager. Provide direct support, education, and skill-building services while advocating for participant preferences throughout the service-planning process. Participate actively in interdisciplinary meetings, care coordination, and clinical discussions. Complete progress notes, assessments, and treatment plans accurately within established timelines. Maintain compliance with regulations and policies while participating in the on-call rotation.
Required Qualifications
- High School Diploma or GED
- New York State Certified Peer Specialist certification
- Personal lived experience with mental health challenges
- Valid New York State Driver's License
Desired Qualifications
- Previous experience in a peer, behavioral health, human services, or other supportive role
- Bilingual Spanish, Haitian Creole, or Yiddish
