Job Summary
Deliver in-home, one-on-one occupational therapy services to older adults under the supervision of an OT, executing individualized treatment plans, guiding therapeutic exercises, and documenting visits in the EMR; coordinate with POAs and referral sources; work within FOX’s Geriatric House Calls model in patients' homes and senior living communities; collaborate with a multidisciplinary team and pursue ongoing professional growth.
Required Qualifications
- Valid Occupational Therapy Assistant license in the state(s) of practice, or eligibility to obtain
- Graduate of an accredited Occupational Therapy Assistant program
- Current CPR certification
