Job Summary
Provide comprehensive home-based occupational therapy to patients as ordered by the attending physician, including education for patients and families, documentation and compliance with Medicare/Federal guidelines; assess functional problems, monitor patient conditions, and collaborate with an interdisciplinary team to improve patient functional status and plan of care. Emphasizes home care context, safety, and caregiver education while delivering skilled OT services and ensuring smooth patient transitions within the home care setting.
Required Qualifications
- Occupational Therapist Assistant (OTA) license in the state of employment
- Minimum one-year experience as a OTA in an acute care setting
- Valid driver's license and auto insurance in your name as a driver
