Job Summary
Assess referred patients requiring rehabilitation services and evaluate their medical condition, functional capabilities, limitations, and restrictions. Establish and administer treatment programs with specific goals determined by the Physician, while documenting patient conditions, progress notes, and discharge documentation. Educate patients and families on appropriate therapy methods and collaborate with all disciplines to provide patient-centered care. Ensure compliance with confidentiality, HIPAA, and Federal Residents Rights. Perform all other duties assigned by the Director of Rehabilitation.
Required Qualifications
- Must have graduated from an accredited college or university
- Must have a valid professional license and registration for state of PA
Desired Qualifications
- Experience with long-term care and/or geriatric care
