Job Summary
Review patient history and data prior to initiating treatment, then collaborate with therapists to execute corrective exercises and treatment plans for inpatients and outpatients. Assist patients and families in discharge planning while documenting all sessions, responses, and plan revisions accurately within the EMR. Participate in patient team and family conferences, attend quarterly retreats, and serve as a resource for peers through mentoring and educational opportunities. Report safety incidents through the Midas/RDE tool, ensure compliance with state and federal regulations, and maintain up-to-date certifications and licenses.
Required Qualifications
- Graduate of an accredited Therapist Assistant Program
- BLS (American Heart Association)
- Current certification in the State of Arizona for the discipline
- Fingerprint Clearance Card application number
- Fingerprint Clearance Card
- Employee will obtain and maintain appropriate clinical competencies for the roles assigned by their departmental leader
- Must attend 75% of department meetings
- Responsible for 100% of department meeting content including reviewing minutes, asking questions as needed, and reviewing handouts
- Completes documentation, including charges within a timely manner
