Job Summary
Deliver occupational therapy treatments under OTR supervision to help patients improve independence with daily living skills following illness, injury, or hospitalization. Implement individualized care plans focused on ADLs and functional independence while collaborating with therapists, nursing staff, and families. Maintain accurate electronic documentation, educate patients and caregivers on therapy programs, and assist with discharge planning and progress tracking. Available for full-time, part-time, or PRN schedules in a skilled nursing facility in Avon, OH.
Required Qualifications
- Certified Occupational Therapy Assistant (COTA / OTA)
- Graduate of an accredited Occupational Therapy Assistant program
- Active Ohio COTA license (or eligible)
- Strong communication and teamwork skills
- Passion for working with geriatric and rehab populations
Desired Qualifications
- Skilled Nursing Facility (SNF), rehabilitation, or post-acute care experience
- Experience with EMR/EHR documentation systems
- Experience in Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Post-Acute Rehabilitation, Assisted Living, Home Health, or Outpatient Therapy settings
