Implement individualized treatment plans to improve functional performance and support independence with daily activities. Provide skilled interventions for self-care, dressing, grooming, bathing, and mobility while monitoring patient progress and communicating changes to supervising therapists. Collaborate with nursing staff, rehabilitation teams, and families to contribute to discharge planning and educate caregivers on therapeutic techniques. Maintain accurate electronic documentation and promote a positive rehabilitation experience for patients in a skilled nursing and post-acute setting.
