Job Summary
Provide home-based skilled occupational therapy services as ordered by the attending physician, delivering patient- and family-centered care in patients’ homes across Jackson, Pike, Adams, and Ross counties. Assess and address functional status, educate patients and families on disease management and equipment use, document accurately following Medicare/Federal guidelines, follow the established Plan of Care and goals, and collaborate with an interdisciplinary team to achieve positive outcomes. Emphasize compassionate communication, safety, and adherence to agency policies while supporting patients’ recovery journey.
Required Qualifications
- Active Occupational Therapy Assistant (OTA) license issued by the state of Ohio at time of application
- Minimum one-year experience as OTA in an acute care setting
- Valid driver's license and auto insurance in the applicant's name
- Capable of all physical demands
- Home care experience preferred
