Job Summary
Evaluate patients, initiate written plans of care, and administer skilled occupational therapy modalities to restore daily living and work skills in their homes. Document services in compliance with federal, state laws, and agency policies. Supervise Certified Occupational Therapy Assistants through record reviews, conferences, and monthly home visits. Coordinate with the multidisciplinary team, physicians, and families to address patient needs, obtain DME, and plan appropriate discharges. Maintain currency of knowledge through continuing education and participate in quality improvement programs. This PRN role reports to the Therapy Manager/Director of Nursing for Home Health.
Required Qualifications
- Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
- Eligible for the National Registration Examination of the American Occupational Therapy Association
- Two (2) years of experience as an Occupational Therapist
- Satisfactory grade on a proficiency examination conducted, approved, or sponsored by the US Public Health Service
- Current licensure as an Occupational Therapist for the State of Ohio
- Minimum of two (2) years' experience as an Occupational Therapist
- Two (2) years home care experience
- One (1) year supervisory experience
- Current CPR card
- Current health certificate/physical examination and TB testing results
- Valid OH Driver's license
- Good driving record
