Job Summary
Evaluate patients and initiate written plans of care, then re-evaluate and update as needed. Provide skilled occupational therapy modalities to develop, recover, or maintain daily living and work skills using compensatory techniques. Document all services in compliance with federal, state laws, and agency policies. Supervise Certified Occupational Therapy Assistants through record reviews, conferences, and home visits every 30 days. Collaborate with physicians, families, and the multidisciplinary team to coordinate DME, ensure timely status updates, and plan appropriate discharges with community resource referrals. Maintain currency through continuing education and participate in quality improvement programs while addressing patient cultural practices.
Required Qualifications
- Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
- Eligible for the National Registration Examination of the American Occupational Therapy Association
- Two (2) years of experience as an Occupational Therapist
- Satisfactory grade on a proficiency examination conducted, approved, or sponsored by the US Public Health Service
- Current licensure as an Occupational Therapist for the State of Ohio
- Minimum of two (2) years' experience as an Occupational Therapist
- Two (2) years home care experience
- One (1) year supervisory experience
- Current CPR card
- Current health certificate/physical examination and TB testing results
- Valid OH Driver's license
- Good driving record
