Certified Occupational Therapist (COTA)

Certified Occupational Therapy Assistant (COTA) works under licensed OT supervision to help residents develop, recover, and maintain activities of daily living and functional skills. Provides therapeutic interventions, documents progress in the EMR, and collaborates with the interdisciplinary care team to support positive clinical outcomes and resident satisfaction. Responsibilities include providing safe, effective care per care plans and physician orders, administering medications and treatments, assessing residents, mentoring LPNs and CNAs, ensuring infection control and safety, and maintaining HIPAA-compliant documentation. Qualifications include carrying out individualized OT treatment plans, helping residents improve mobility, self-care, and independence, documenting progress, communicating changes to the supervising OT and team, educating residents and families about therapy programs, and participating in care plan meetings. The role is on-site in Elkhorn, WI with flexible schedule options (Full-time/Part-time/Casual) and opportunities for career growth within a 60-bed skilled nursing community.
