Job Summary

Implement occupational therapy treatment plans under supervision, delivering skilled interventions targeting ADLs, IADLs, neuromuscular re-education, and cognitive retraining. Monitor patient response, report clinical concerns, and assist with data collection for progress assessments. Instruct patients and caregivers on home programs and safe equipment use while assisting with transfers, positioning, and therapeutic handling. Support discharge planning, transport patients between areas, and maintain accurate documentation in compliance with state laws and payer standards. Participate in team meetings, quality assurance, and performance improvement initiatives while upholding HIPAA, OSHA, and infection control protocols.

Required Qualifications

  • Current, active, and unrestricted Certified Occupational Therapist Assistant license in the State of California
  • Graduate of an accredited Occupational Therapy Assistant program
  • Successful completion of the NBCOT examination for Occupational Therapy Assistants
  • Current CPR/BLS certification with AHA
  • Ongoing compliance with continuing education requirements established by the California Occupational Therapy Board
  • Ability to assist with patient transfers, upper extremity positioning and therapeutic handling, lifting and repositioning patients with or without assistive devices
  • Ability to perform prolonged standing, walking, bending, reaching, kneeling, and stair navigation
  • Ability to assist in resident evacuation during emergency situations
  • Excellent verbal and written communication and interpersonal skills
  • Demonstrated maturity, composure, and professionalism in dynamic, fast-paced clinical environments
  • Strong organizational skills and the ability to manage multiple priorities under supervision
  • A team-first mindset and a genuine passion for skilled nursing and post-acute care
  • Ability to lift 50 lbs

Desired Qualifications

  • Willingness to obtain CPR/BLS certification per facility policy
