Job Summary
Provide direct occupational therapy rehabilitation to patients under the supervision of a licensed OT in a Life Care Centers of America SNF setting. Follow established OT treatment plans, document progress, and utilize therapy software. Collaborate with an interdisciplinary team to plan for patient needs, ensure patient rights and safety, and maintain confidentiality. Requires NBCOT certification and active state licensure; CPR certification at hire or during orientation. New grads welcome; one year post-acute experience preferred. Essential functions include implementing treatment plans, charting, communication with team, and assisting in patient evacuation as needed.
Required Qualifications
- NBCOT certified upon hire
- Graduate of an accredited OT program (ASOT)
- Currently registered/licensed in applicable State; active license in good standing
- CPR certification upon hire or during orientation; CPR must remain current
- One year experience in post-acute care or related setting preferred
- Proficient in Microsoft Word, Excel, and email
- Liaisons with patients, families, and other departments to plan for patient needs
- Ability to follow treatment plans and participate in interdisciplinary team
- Demonstrates good body mechanics and safety practices
- Communicates effectively; maintains confidentiality
- Adheres to company policies and code of conduct
- Demonstrates integrity and professionalism
