Job Summary
Certified Occupational Therapist Assistant (COTA) for home health. Provide rehabilitation services under supervision, implement and modify the Plan of Care based on an OT assessment, perform routine visits and re-evaluate therapy needs, communicate findings to supervisors and care team, educate patients and caregivers on therapy and home exercise programs, ensure compliance with regulatory and payer requirements, coordinate with clinical staff, and assist in preventing hospitalizations/ER visits. Requires graduation from an OT curriculum, current COTA certification, willingness to travel within the service area (up to ~50%), and the ability to lift and transfer patients (up to 50 pounds). Full-time role with potential travel and standard benefits; equal opportunity employer.
Required Qualifications
- Graduate of a certified occupational therapy curriculum established by the Occupational Therapy Association
- Current, unrestricted COTA certification in the state of work
- Ability to travel within branch/office coverage area (up to 50%)
- Ability to frequently lift, push, pull, and transfer patients up to 50 pounds
- Must have dependable vehicle and valid driver's license
