Job Summary
Provide personal care including bathing, grooming, oral care, toileting, dressing, and skin care while assisting with mobility, transfers, ambulation, and range-of-motion exercises. Take and document vital signs, observe patient conditions, monitor skin integrity, and accurately document care in accordance with agency standards. Support feeding and hydration, prepare meals as needed, and promote a clean, safe, and comfortable living environment. Communicate with empathy, support family members and caregivers, and collaborate with nurses and the interdisciplinary team to ensure continuity of care. Follow all hospice regulations, policies, and scope-of-practice guidelines while participating in ongoing quality and performance improvement initiatives.
Required Qualifications
- Active Certified Nursing Assistant (CNA) certification in Minnesota
- Current CPR certification
- Valid driver's license with reliable transportation

- Previous hospice, home health, or senior care experience
- Strong understanding of hospice philosophy and end-of-life care
- Compassionate, patient-centered approach with strong communication skills
