Certified Nursing Assistant - Transitional Care

Provides direct patient care under the supervision of a nurse in Transitional Care. Greets and orients patients, supports care team, and fosters a positive patient experience. Maintains patient safety through intentional rounding, regular updates to the care board, and adherence to documentation standards in the EMR. Assists nursing staff with patient assessments, personal care, and workflow tasks including unit readiness, diet/verifications, patient transport coordination, and telemetry observation. Requires critical thinking, ability to work under minimal supervision, and the capacity to function in a high-stress hospital environment. Education and licensure requirements include a Kentucky State Registered Nurse Assistant (SRNA) credential upon hire and BLS within 90 days of employment. Weekend coverage required; 0.9 FTE.
