Job Summary
Provide personal care including bathing, dressing, oral care, feeding, and elimination while assisting with ambulation, positioning, transfers, and ADLs. Travel to and care for multiple hospice patients per day in their residences, measuring and documenting vital signs and observations as directed. Observe and report changes in patient condition or home safety concerns, following RN care plans and agency policies while maintaining patient confidentiality and HIPAA compliance. Support patient comfort through socialization and emotional presence. Requires current CNA license, one year of direct patient care experience, and listing on the Nurse Aide Registry.
Required Qualifications
- Current CNA license in the state which you are applying
- Minimum 1 year of direct patient care experience
- Listed on the Nurse Aide Registry with no findings of abuse or neglect
Desired Qualifications
- High school diploma or equivalent
