Job Summary
Provide resident care to assigned patients in accordance with assessments, care plans, and nursing management direction. Coordinate proper care types with licensed staff while strictly following company policies, safety guidelines, and resident assessments. Work independently on a day-to-day basis with strong support from a patient care team including nurses, social workers, chaplains, and volunteers. Maintain a valid driver's license and auto insurance for reliable transportation. This role supports Brighton/Suncrest Hospice's goal of changing care expectations through increased clinical visits and lowered clinician caseloads within a CHAP-certified environment.
Required Qualifications
- Must be willing to drive with reliable transportation, valid driver's license, and auto insurance
- Certified Nursing Assistant (CNA) certification in the state(s) of practice
- Must be at least 18 years of age
