Job Summary
Provide resident care to assigned individuals in accordance with assessments, care plans, and nursing management directives. Coordinate proper care types with licensed staff while strictly adhering to company policies, procedures, and safety guidelines. Collaborate daily with nurses, social workers, chaplains, volunteers, and leadership to support patient needs. Work independently on a day-to-day basis with reliable transportation, valid driver's license, and auto insurance. Maintain CNA and/or HHA certification in the state of practice.
Required Qualifications
- Must be willing to drive with reliable transportation, valid driver's license, and auto insurance
- Certified Nursing Assistant (CNA) certification in the state(s) of practice
- Home Health Aide (HHA) certification in the state(s) of practice

- Able to work independently and on your own from a day-to-day basis
