Certified Nursing Assistant

Provide each assigned resident with routine daily nursing care and services in accordance with their assessment, care plan, and supervisor directives. Perform all tasks per established policies, review care plans daily to identify changes in resident routines, and inform the Nurse Supervisor of any condition changes for documentation. Ensure all notes accurately reflect that the care plan is being followed while maintaining flexibility to work effectively with residents, personnel, and support agencies. This role supports Health Concepts' six skilled nursing facilities across Rhode Island, offering opportunities at locations including Providence, East Providence, Warwick, West Kingston, Newport, and Burrillville.
